What Does Embryo Grading Mean for Your IVF Journey?

Posted on November 2, 2026 by Inception Fertility

If you're going through IVF, you've probably heard your care team mention embryo grading — and it can feel like a foreign language at first. Understanding what those numbers and letters actually mean can help you feel more informed and less anxious as you move through treatment.

How Embryos Are Evaluated

After fertilization, your embryos are monitored in the lab over several days as they grow and develop. Embryologists assess them at different stages — typically on days three and five or six — looking at factors like the number of cells, how evenly those cells are dividing, and the overall appearance of the embryo. By day five or six, embryos that have reached the blastocyst stage receive a more detailed grade based on three things: expansion of the blastocyst, the quality of the inner cell mass (the cells that become the baby), and the quality of the trophectoderm (the cells that become the placenta).

What the Grades Actually Mean

A common grading scale uses a number for expansion (typically 1 through 6) followed by two letters — one for the inner cell mass and one for the trophectoderm — with A being the highest quality and C being the lowest. So an embryo graded 4AA, for example, is an expanded blastocyst with excellent cell quality in both areas. That said, embryos graded 3BB or even lower have successfully led to healthy pregnancies. Grading gives your team useful information, but it's not a guarantee in either direction.

Grading Is a Tool, Not a Crystal Ball

It's completely natural to feel attached to the idea of having "perfect" embryos, but it's worth knowing that grading is just one piece of a much bigger picture. Your doctor considers your age, medical history, the results of any preimplantation genetic testing, and other factors when deciding which embryo to transfer and when. An embryo that looks great on paper may not implant, and one with a more modest grade might be the one that works. The grading system helps your care team make thoughtful, personalized decisions, but it's a guide, not a verdict.
